Home Care Services

 

Live-in Home Care

All of our home health aides are supervised by a registered nurse. The supervising RN regularly visits each of our patients to assess progress, consult with the family, and make any necessary adjustments to the plan of care.

We have a minimum of three service hours per day. Patients and families can choose to have around the clock (live-in) care, or pick their desired hours of service.

We will typically introduce a patient to a number of different caregivers, from which they can choose the best match.

 

Nursing Visits

Specialist Services

 

Home Health Care encompasses a wide range of care services provided to patients in homes, in nursing homes, or in long term care facilities. With Home & Hospital, all home health care is performed by licensed, bonded, and insured certified home health aides. These services include:
